CDCP Eligibility & Co-Pay Calculator

This includes coverage through work, a family member, pension benefits, or a purchased/private plan.
Are you a Canadian resident for tax purposes?
Did you file your most recent Canadian tax return?
If you have a spouse or common-law partner, they generally need to have filed as well.
Adjusted family net income
Use adjusted family net income from your most recent tax return. This is generally based on line 23600 for you and your spouse/common-law partner, adjusted for UCCB and RDSP amounts. Not the same as gross household income.
Optional: estimated CDCP established fee
Use this only if your dental office gives you an estimate based on CDCP established fees. Co-pay is calculated on CDCP fees, not always the clinic's full fee.
